COVID Testing Information for Surgical Patients
Fully vaccinated patients are not required to take and submit a COVID test prior to surgery. Please email proof of vaccination to [javascript protected email address]. Unvaccinated patients are still required to submit a negative COVID test (antigen and antibody tests are unacceptable). Please follow the directions below.
